What it is and when to use it
The ISTH score quantifies overt DIC from four laboratory parameters: platelets, a fibrin-related marker, prothrombin time and fibrinogen.
How to interpret it
A score of 0 means none of the four criteria is abnormal; overt DIC is not supported at this time. A score of 1–4 is not compatible with overt DIC but is suggestive of non-overt DIC; repeat the score in 1–2 days if the underlying disorder persists. A score of 5 or more is compatible with overt DIC and should be repeated daily.
Limitations and when not to use it
Applicable only when the patient has an underlying disorder known to be associated with DIC. It does not replace clinical judgement or treatment of the underlying cause.
